If covid positive 

• BMC will call you and send their sanitation team to "disinfect" your house with sodium hydrochloride on the same day the test reports reach them . 
• They will spray it on your bed and floor (December 2020) 
• The bmc doctor will call and tell you the options . 
• For home isolation the bmc will need a signed consent form (they will send it on your whatsapp )
• You need to print it out and take a photo and send it to them 
• You also need to ask your family doctor for a prescription and you need to send it to the bmc doctor via whatsapp . You also need to email the central government the two documents. id will be given by the bmc doctor .

Registration Process
1) Enter Patient’s Mobile Number and log in with the OTP sent on this mobile number.
2) Select your Preferred Location for Sample Collection.
3) Select Available Slots for Sample Collection.
4) Fill up the COVID-19 TRF with complete and accurate details.
5) Upload ID Proof(mandatory), Doctor’s prescription, Hospital Discharge report etc.
6) Click on 'Confirm Booking'
7) A token number will be generated. Save the token number on your phone to display at the collection site.

Sample Collection Process
1) Read the instructions carefully as displayed by the medical staff.
2) Two swabs will be collected; from the Nose and Throat.
3) Please collect the hand out for report details and leave the site.
4) Do not visit the site or any Suburban Diagnostics centre for report collection.
